1. (1)  Beginning January 1, 2025, each surplus line broker shall file a quarterly report detailing surplus-lines transactions occurring in this state in the previous calendar quarter. The filing schedule shall be as follows:

   1st Quarter (January 1 – March 31): filing due May 30;         2nd Quarter (April 1 – June 30): filing due August 30;         3rd Quarter (July 1 – September 30): filing due November 30;   4th Quarter (October 1 – December 31): filing due January 30.

2. (2) The forms used in connection with the Surplus Line Insurance Law are on the Department of Insurance's web page at http://www.aldoi.gov. The right to amend, rescind, or add to the posted forms is reserved without amending this rule.
