1. (1) Notification Prior to Performance of Activity. An operator shall notify theSupervisor prior to performing any of the following activities:

   1. (a) Setting surface casing, see Rule 400-1-4-.03(1);
   2. (b) Running intermediate or production casing, see Rule 400-1-4-.03(1);
   3. (c) Perforating, see Rule 400-1-4-.03(1);
   4. (d) Drillstem testing, see Rule 400-1-4-.03(1);
   5. (e) Wireline logging or surveying, see Rule 400-1-4-.03(1);
   6. (f) Coring, see Rule 400-1-4-.03(1);
   7. (g) Pressure testing, see Rule 400-1-4-.09(4) or Rule 400-4-2-.01(2)(d);
   8. (h) Cleaning tanks, see Rule 400-1-6-.12;
   9. (i) Turnaround operations for processing facilities, see Rule 400-1-7-.01(2)(c);
   10. (j) Turnaround operations for plants, see Rule 400-1-7-.01(3)(d); and
   11. (k) Initiating a gathering line construction operation, see Rule 400-1-8-.03(5)(a).
2. (2) Witness of Activities. The Supervisor may send a duly authorized representative tothe location to witness activities in section (1) above.

3. (3) Notification Subsequent to Occurrence of Activity. An operator shall notify theSupervisor when the following occurs:

   1. (a) Sale, assignment, or acquisition of any well and associated facilities, see Rule400-1-2-.05;
   2. (b) Loss of a radioactive logging source, see Rule 400-1-4-.05(1);
   3. (c) Encountering unexpected wellbore conditions during recompletion or reworking, seeRule 400-1-6-.06(2);
   4. (d) Repairing or replacing damaged gathering lines, see Rule 400-1-8-.03(5)(a);
   5. (e) Fire, spill, leak, or blow out, see Rule 400-1-9-.01; and
   6. (f) Hydrogen sulfide concentration reaches fifty (50) parts per million in air, see Rule400-1-9-.02(4)(d)3.
