The recipient shall immediately notify the Department in writing of events or proposed changes that may require a modification to the assistance agreement, including:

1. (a) Changes in plans and specifications for the project;
2. (b) Changes to the scope or objectives of the project;
3. (c) Significant, changed conditions at the project site;
4. (d) Significant changes to the construction schedule;
   and
5. (e) Changes that may substantially change the total
   cost of the project
