1. (1) Reasons why technical assistance is needed:

   1. (a) Problems with incubator
   2. (b) Unique opportunities
   3. (c) Opportunities for networking
   4. (d) Other.
2. (2) Background of expected provider.
3. (3) Proximity of expected provider.
4. (4) Expected results from technical assistance.
