Any dentist practicing in the State of Alabama must notify the Board of Dental Examiners of Alabama (Board) of any mortality or significant injury occurring during or directly related to a dental procedure or treatment performed by a dentist or in which a dentist participated in any manner whether occurring in an office, hospital, or other outpatient treatment facility within seven (7) business days from the date on which the dentist first becomes aware of the mortality or significant injury. A significant injury is defined as physical injury that results in hospital admission.In addition, the dentist shall submit a complete report to the Board within thirty (30) business days from the date on which the dentist first becomes aware of the referenced occurrence which must include, at a minimum, the following:Description of the dental procedure.Description of preoperative physical condition of the patient, including recorded vitals.List of drugs and dosages administered.Description, in detail, of techniques utilized in administering the drugs used.Description of the adverse occurrence.Describe, in detail, symptoms of any complications to include, but not be limited to, the onset and type of symptoms exhibited by the patient.Treatment instituted on the patient.Response of the patient to the treatment.Author: Board of Dental Examiners of Alabama
