A cooperative may change its registered office or change its registered agent, or both, upon filing in the office of the commissioner a statement setting forth

1. (1) the name of the cooperative;
2. (2) the address of its registered office;
3. (3) if the address of its registered office is changed, the address of the new registered office;
4. (4) the name of the registered agent;
5. (5) if its registered agent is changed, the name of its new registered agent;
6. (6) that the address of its registered office and the address of the business office and its registered agent, as changed, will be identical;
7. (7) that such change was authorized by resolution adopted by its board of directors.
