1. (a) An insurer subject to this chapter shall file, in a format prescribed by the director, notice with the division not later than 10 days after the initiation or renewal of the employer's workers' compensation insurance.
2. (b) A self-insured employer or insurer subject to this chapter, unless exempted, shall file, in a format prescribed by the director, notice with the division not later than 10 days after changing insurance adjusting services.
