Every person employing a minor shall obtain and have on file proof of the minor's age acceptable to the commissioner. Examples of acceptable proof of age include, but are not limited to, a copy of

1. (1) birth certificate;
2. (2) Bureau of Indian Affairs census record;
3. (3) passport;
4. (4) driver's license;
5. (5) authenticated school records;
6. (6) federal age certificate;
7. (7) baptismal certificate;
8. (8) military dependent identification;
9. (9) family court records;
10. (10) affidavit of physician.
