(a) An ICF/IID shall assign a recipient an interdisciplinary team, to be directly involved in the treatment of the recipient, and to develop, implement, monitor, and evaluate the habilitative plan of care required under 7 AAC 140.610. An interdisciplinary team must include, at a minimum,
