1. (a) The department will make a finding of eligibility or ineligibility for every identifiable application for Medicaid that it receives under 7 AAC 100.004, unless

   1. (1) a written request to withdraw the application is submitted by an individual authorized to sign the application under 7 AAC 100.004(b);
   2. (2) the department has factual information confirming the death of the applicant; or
   3. (3) the applicant cannot be located.

2. (b) For the purposes of this section, an applicant cannot be located if

   1. (1) the post office returns mail sent to the mailing address shown on the application and indicates no known forwarding address;
   2. (2) the applicant has not reported a new address; and
   3. (3) reasonable attempts to locate the applicant by using information on the application form have failed.
