For each category of service provided by a behavioral health services provider, that provider must

1. (1) have in writing

   1. (A) a service description;
   2. (B) a service philosophy; and
   3. (C) service goals;
2. (2) establish procedures for crisis intervention, including screening recipients for risk to self or others;
3. (3) provide clinical supervision to all personnel providing clinical or direct services to a recipient; and

4. (4) conduct regular quality assurance reviews that

   1. (A) monitor the quality of the service;
   2. (B) monitor the appropriateness of service; and
   3. (C) are used to identify training needs and improve the quality of the service.
