1. (a) In addition to the notice of changes required by AS 47.32.200, and the notification requirements of 7 AAC 10.925, a child care facility shall report to the department

   1. (1) within 14 days, the addition, for 45 or more days in a 12-month period, of a member of the licensee's household; and

   2. (2) not later than 30 days before one of the following planned changes is expected to occur:

      1. (A) a change in the person operating the facility;
      2. (B) a change in the name of the person operating the facility;
      3. (C) a change in the name of the facility;
      4. (D) a change of administrator;
      5. (E) a change in the age of children served;
      6. (F) a change in the number of children in care authorized by the facility's license;
      7. (G) a change in hours of operation;
      8. (H) the deletion or addition of a specialization under 7 AAC 57.800 - 7 AAC 57.810;
   3. (3) the permanent departure of an individual, age 16 or older, associated with a child care facility in a manner described in 7 AAC 10.900(b); and
   4. (4) a change in the plan of operation or other information included in the application submitted under 7 AAC 57.030.

2. (b) A child care facility shall immediately report to the department the following occurrences in the facility:

   1. (1) the death of a child while in care;
   2. (2) a serious injury or illness of a child while in care that requires attention by medical personnel outside of the facility;
   3. (3) a fire or other emergency situation that affects the facility, reported as required by 7 AAC 10.1010(i);
   4. (4) an unplanned change in any occurrence listed in (a) of this section; and
   5. (5) suspension or expulsion of a child in care for challenging or out-of-control behavior.

3. (c) A child care facility shall immediately report the following incidents involving a child in care to the child's parent:

   1. (1) the death of a child while in care;
   2. (2) an injury or illness of a child in care that requires attention by medical personnel outside of the facility;
   3. (3) the exposure of a child to a contagious condition such as lice or scabies or to a communicable disease other than a cold.
