For services furnished on or after July 1, 2004, the department will not make a payment under the CAMA program for any expense that is

1. (1) for major medical care;
2. (2) for nursing facility care;
3. (3) for items and services not properly prescribed or determined necessary by a health care provider enrolled with the department;
4. (4) for a person in the care and custody of a federal, state, or local correctional facility;
5. (5) for an elective procedure;
6. (6) for services furnished at no charge to an Indian Health Service beneficiary by an Indian Health Service facility or a facility funded by the Indian Health Service;
7. (7) not directly related to the treatment of a medical condition listed in 7 AAC 48.525(b);
8. (8) for prescriptions after the first three are filled within a calendar month;
9. (9) for transportation; or
10. (10) for physician services if the physician services are furnished in an inpatient hospital or in a nursing facility.
