- (a) A person applying for financial assistance under this chapter must apply on a form approved by the committee. The committee may deny an application for failure to complete the application form or otherwise to provide information required by the committee.
- (b) Application forms, along with the formula used to calculate the applicant's share, described in 7 AAC 48.070, are available at local offices of the division of public assistance, at hospitals in the state, and at the offices of the division of medical assistance.
