1. (a) An individual may apply for assistance under 7 AAC 47.800 - 7AAC 47.890 by

   1. (1) applying on a form provided by the department; the department may provide an abbreviated form to an individual who is eligible under 7 AAC 47.805(b); and
   2. (2) certifying on a form provided by the department to the accuracy and truthfulness of information provided under (1) of this subsection.
2. (b) Application may be made by the individual seeking assistance or by a responsible person acting on the individual's behalf.
3. (c) If an individual wishes to continue to receive assistance each month under 7 AAC 47.800 - 7 AAC 47.890, the individual is not required to submit a new application under (a) of this section.
4. (d) If the individual discontinues assistance under 7 AAC 47.800 - 7 AAC 47.890, the individual may reapply by submitting the forms specified in (a) of this section.
