1. (a) An individual may apply for assistance under AS 47.45.301 - 47.45.309 and 7 AAC 47.547 - 7 AAC 47.599 by

   1. (1) submitting an application;
   2. (2) certifying on a form provided by the department to the accuracy and truthfulness of information provided under (1) of this subsection; and
   3. (3) providing documentation requested by the department to verify information provided under (1) of this subsection.
2. (b) Application may be made by the individual seeking assistance or by a responsible person acting on the individual's behalf.
3. (c) If the individual discontinues assistance under AS 47.45.301 - 47.45.309 and 7 AAC 47.547 - 7 AAC 47.599, the individual may reapply by submitting the forms specified in (a) of this section.
