A person described in 7 AAC 47.330 may apply for assistance under 7 AAC 47.300 - 7 AAC 47.535 on a form provided by the department. The department, or a responsible person acting on an applicant's behalf, may assist the applicant in completing the application. The application must include a physician's written report completed by the applicant's physician, physician assistant, or advanced practice registered nurse in a format provided by the department.
