In this chapter, unless the context requires otherwise,

1. (1) "certified copy" means a copy of the original document that has been certified to be a true and correct copy of the original

   1. (A) by the person who signed the original document;
   2. (B) by a notary public or other person authorized to administer oaths under AS 09.63.010; or
   3. (C) if a notary public or other person authorized to administer oaths under AS 09.63.010 is unavailable, in accordance with the provisions of AS 09.63.020;
2. (2) "debilitating medical condition" has the meaning given in AS 17.37.070;
3. (3) "department" means the Department of Health;
4. (4) "minor" means a patient under the age of 18 who has not had the disabilities of minority removed under AS 09.55.590;
5. (5) "parent" means a biological or adoptive parent who retains the legal rights to make medical decisions regarding the care of the minor;
6. (6) "patient" has the meaning given in AS 17.37.070;
7. (7) "physician" has the meaning given in AS 17.37.070;
8. (8) "primary care-giver" has the meaning given in AS 17.37.070;
9. (9) "qualified debilitating medical condition" has the meaning given "debilitating medical condition" in AS 17.37.070;
10. (10) "qualified physician" has the meaning given "physician" in AS 17.37.070;
11. (11) "registry identification card" has the meaning given in AS 17.37.070;
12. (12) "written documentation" has the meaning given in AS 17.37.070.
