In this chapter, unless the context requires otherwise,
(1) "certified copy" means a copy of the original document that has been certified to be a true and correct copy of the original
- (A) by the person who signed the original document;
- (B) by a notary public or other person authorized to administer oaths under AS 09.63.010; or
- (C) if a notary public or other person authorized to administer oaths under AS 09.63.010 is unavailable, in accordance with the provisions of AS 09.63.020;
- (2) "debilitating medical condition" has the meaning given in AS 17.37.070;
- (3) "department" means the Department of Health;
- (4) "minor" means a patient under the age of 18 who has not had the disabilities of minority removed under AS 09.55.590;
- (5) "parent" means a biological or adoptive parent who retains the legal rights to make medical decisions regarding the care of the minor;
- (6) "patient" has the meaning given in AS 17.37.070;
- (7) "physician" has the meaning given in AS 17.37.070;
- (8) "primary care-giver" has the meaning given in AS 17.37.070;
- (9) "qualified debilitating medical condition" has the meaning given "debilitating medical condition" in AS 17.37.070;
- (10) "qualified physician" has the meaning given "physician" in AS 17.37.070;
- (11) "registry identification card" has the meaning given in AS 17.37.070;
- (12) "written documentation" has the meaning given in AS 17.37.070.
