To obtain recertification, an EMD must, not more than 90 days before or six months after expiration of the EMD's certification,

1. (1) apply for recertification to the department;
2. (2) provide evidence of a valid CPR credential;
3. (3) provide written approval from the medical director for the emergency medical dispatcher services;
4. (4) repealed 8/16/2002; and
5. (5) pay a non-refundable application fee of $10 to the department.
