Alaska Admin. Code tit. 7, § 13.085 – Program evaluation

(a) A facility shall develop procedures necessary to evaluate the program and shall conduct an evaluation annually.
(b) The evaluation must include
1. (1) information about demographic characteristics;
2. (2) assessment of community and client reaction to services, which may include questionnaires, surveys, or board reports; and
3. (3) progress report for the five-year plan for increasing self-sufficiency.
