An applicant for a certificate of need must include with the application a nonrefundable application fee as follows:

1. (1) for an activity valued at $2,500,000 or less, $2,500;
2. (2) for an activity valued at more than $2,500,000, a fee equal to .1 percent of the estimated cost, up to a maximum fee of $75,000.
