1. (a) Not later than May 1 of each year, a school participating in the department reading program shall submit to the department a K - 3 intensive school reading improvement plan.
2. (b) The department will make a K - 3 intensive school reading improvement plan template available on its website.

3. (c) The participating school shall complete the plan in partnership with the assigned reading specialist. The plan must include

   1. (1) a description of the reading program and reading intervention services to be provided to students;
   2. (2) goals for reading improvement with measures of interim progress;
   3. (3) a description of the roles and responsibilities of the school, the reading specialist, and the department;
   4. (4) a description of professional development plans for district staff involved in implementing the plan;
   5. (5) a description of reading improvement home supports; and
   6. (6) a description of the daily school schedule, redesigned as provided under AS 14.30.770(b)(2).
4. (d) Submission of the K - 3 intensive school reading improvement plan satisfies the requirement that a district enter into a written agreement with the department as provided under AS 14.30.770(b)(2).
