Every application and every application form prescribed by the commission must comply with the applicable requirements of 3 AAC 48.090 - 3 AAC 48.100, including those concerning filing, type of organization, general form, execution, and verification. However, the factual data set out in the numbered paragraphs and the exhibits attached to, and made a part of, an application will, in the commission's discretion, be prescribed by general order.
