Format of tariff sheets

Every tariff sheet must be legible and formatted to print eight and one-half by 11 inches in size and, except as provided in 3 AAC 48.360(d), include
- (1) the number of the utility or pipeline carrier's certificates of public convenience and necessity;
- (2) the tariff sheet number;
- (3) the tariff sheet revision number;
- (4) the name of the utility or pipeline carrier;
- (5) the tariff advice number; and
- (6) the proposed effective date.
