A health care insurer shall
- (1) monitor all utilization review activities carried out by, or on behalf of, the health care insurer;
- (2) ensure that the requirements of 3 AAC 28.900 - 3 AAC 28.918 are met; and
- (3) ensure that appropriate personnel have operational responsibility for the conduct of the health care insurer's utilization review program.
