A continuing education course may not be advertised, sponsored, or offered in this state unless the director has approved the course and the advertisement includes

1. (1) the continuing education provider's name as approved by the director;
2. (2) the course title as approved by the director;
3. (3) a brief description of the content of the course;
4. (4) the number of credit hours approved by the director; and

5. (5) for a contact class course,

   1. (A) the time and date the course will be offered; and
   2. (B) the location where the course will be offered.
