1. (a) If the department denies, inactivates, or significantly modifies a loan request, an applicant may file a written request for reconsideration not later than 30 days after receiving notice of the department's decision.

2. (b) The department will reconsider the applicant's request if the applicant

   1. (1) shows that there has been a substantial change in the circumstances leading to the department's decision;
   2. (2) provides additional relevant information to the department that was not initially available; or
   3. (3) shows that the department made an administrative error.
