A licensee terminating deferred deposit advance operations must

1. (1) provide electronic notice to the department through the registry of a decision to cease operations or the cessation of operations not later than seven days after that decision or cessation; and
2. (2) submit a completed request for surrender; and
3. (3) submit reports as required under 3 AAC 11.050 through the date of termination of operations.
