A creditor may not use any false, deceptive or misleading representation or means in connection with the collection of any debt, including but not limited to the following:

1. (1) any false representation

   1. (A) that the lender is vouched for, bonded by, or affiliated with the United States or any state;
   2. (B) of the character, amount or legal status of any debt;
   3. (C) of any services rendered or compensation due the creditor for the collection of any debt;
   4. (D) that documents are not legal process forms or do not require action by the debtor;
2. (2) communicating or threatening to communicate to any person credit information which is known to be false or, if a debt is disputed, failing to communicate that fact when giving credit information.
