An applicant for an arts education incentive grant must

1. (1) complete an arts education incentive grant application provided by the council in the guidelines and application form, adopted by reference in 20 AAC 30.983(10), for an arts education incentive grant; and

2. (2) submit the following items, so that each is received or postmarked not later than the deadline established in the guidelines and application form described in (1) of this section:

   1. (A) the completed application;
   2. (B) any supporting materials requested in the guidelines and application form.
