The exact surface location of an abandoned onshore well must be shown by a well abandonment marker plate that

1. (1) is constructed of steel that is at least 1/4 inch thick;
2. (2) is welded to, and covers, the outermost casing string, including the conductor casing;
3. (3) does not extend beyond the outermost casing string; and

4. (4) contains the following information bead-welded directly to the marker plate:

   1. (A) The name of the operator that plugged and abandoned the well;
   2. (B) the number assigned by the commission to the Permit to Drill (Form 10-401) for the well under 20 AAC 25.040(b);
   3. (C) the name designated for the well by the operator under 20 AAC 25.005(f);
   4. (D) the API number for the well, with any suffix appended to the number under 20 AAC 25.005(f).
