If a license issued under this chapter is lost or destroyed, the person whose name appears on the license may apply for a duplicate license by submitting to the department

1. (1) an application on a form or in a manner prescribed by the department containing the information regarding the circumstances of how the license was lost or destroyed; and
2. (2) the duplicate license fee of $15.
