An agreement for payment of damages, as provided for in AS 28.20.120(a), must

1. (1) clearly state the names of all the parties to the agreement;
2. (2) be signed by all the parties to the agreement; and
3. (3) be notarized by a notary public or witnessed by a division employee using the settlement form provided by the department.
