A group or nongroup entity shall report each contribution from each contributor in an ongoing payroll deduction program or other automatic payment plan in each reporting period for each contributor by providing the

1. (1) name and address of the contributor;
2. (2) repealed 1/16/2015;
3. (3) repealed 1/16/2015;
4. (4) principal occupation and employer of the contributor, unless the cumulative amount received from that contributor in a calendar year is $100 or less; and
5. (5) amount and frequency of each deduction or payment.
