Cause for terminating a permit before the end of its term, or for denying a permit renewal application, includes the following:

1. (1) the permittee's noncompliance with any condition of the permit;
2. (2) the permittee's failure to fully disclose all relevant facts in the application or during the permit issuance process, or the permittee's misrepresentation of any relevant fact at any time;
3. (3) the department's determination that the permitted activity endangers human health or the environment and can only be regulated to acceptable levels by permit modification or termination;
4. (4) a change in any condition, such as closure of a plant or termination of a discharge by connection to a POTW, if the change requires either a temporary or permanent reduction or elimination of any discharge.
