Alaska Admin. Code tit. 18, § 80.443 – Level 1 assessment requirements

(a) The requirements for a Level 1 assessment under 40 C.F.R. 141.859, adopted by reference in 18 AAC 80.010(a), apply to a public water system. In addition, the department will require the Level 1 assessment report submitted to the department by the owner or operator to
1. (1) be completed on a current form provided, and in a format approved, by the department;
2. (2) accurately describe the results of the Level 1 assessment; and
3. (3) be signed by the person who conducted the assessment, subject to the report certification requirements of 18 AAC 80.1900.
(b) The requirements of 18 AAC 80.050 apply to a Level 1 assessment.
