1. (a) A permittee or an operator shall obtain a bingo game prize receipt at the time the prize is won and retain the receipt as required under 15 AAC 160.870(a).
2. (b) Bingo game prize receipts must be consecutively numbered.

3. (c) A bingo game prize receipt must include

   1. (1) the location, the name of the permittee, and, if applicable, the operator;
   2. (2) the date the receipt was issued;
   3. (3) the bingo game number;
   4. (4) the printed name of each prize winner;
   5. (5) a description of the prize or a copy of the invoice issued when the prize was purchased; and
   6. (6) the signature of each prize winner.
4. (d) Repealed 11/10/96.
