1. (a) The quarterly report required under AS 05.15.083(a) must be on a form provided by the department and the report must include, for each authorizing permittee on whose behalf gaming activity was conducted during the quarter,

   1. (1) the amount of gross receipts for each gaming activity;
   2. (2) the value of prizes awarded for each gaming activity;
   3. (3) the total value of prizes including donations;
   4. (4) the amount of municipal, state, and federal taxes paid;
   5. (5) the adjusted gross income;
   6. (6) the expenses under AS 05.15.160(a);
   7. (7) the total net proceeds;
   8. (8) other information required by AS 05.15.083(a); and
   9. (9) additional information that the department determines to be necessary to determine the amount of proceeds due the permittee.

2. (b) The annual report required under AS 05.15.083(b) must be on a form provided by the department. The report must include, for each authorizing permittee on whose behalf gaming activity was conducted during the year,

   1. (1) the amount of gross receipts for each gaming activity;
   2. (2) the value of prizes awarded for each gaming activity;
   3. (3) the expenses under AS 05.15.160(a);
   4. (4) the amount of net proceeds for each gaming activity;
   5. (5) other information required by AS 05.15.083(b); and
   6. (6) additional information that the department determines to be necessary to determine the amount of proceeds due the permittee.
