1. (a) To apply for an energy rebate, an eligible rebate recipient shall complete an application on a form provided by the corporation and shall certify that the information provided is true and correct.

2. (b) The corporation will review the energy rebate application and ensure that the following conditions are met:

   1. (1) all information requested on the form has been provided;
   2. (2) for a newly constructed dwelling unit, a copy of the energy rating is provided;
   3. (3) for an existing dwelling unit, copies of the energy ratings before and after the improvement are provided;
   4. (4) the rebate amount is correct; and
   5. (5) the applicant is an eligible rebate recipient.
3. (c) For an energy rating rebate, the energy rater or eligible rebate recipient shall complete an application on a form provided by the corporation and shall certify upon it the accuracy of the information provided.
4. (d) The corporation will review the energy rating rebate application and ensure that program conditions are met.
5. (e) For rebates for other energy saving measures, the applicant shall complete an application on a form provided by the corporation and shall certify that the information is true and correct.
