Alaska Admin. Code tit. 13, § 75.060
Annually, the department will require verification from a surviving dependent of continued eligibility to receive payment of medical insurance premium payments. The department will provide a continuing eligibility verification form to each surviving dependent not later than 90 days before the anniversary of the peace officer's or firefighter's death. The surviving dependent must complete and certify the information is correct and complete, and return the verification form to the department not later than 30 days after receiving it. Failure of a surviving dependent to timely certify continuing eligibility may result in termination of payment of medical insurance premium payments.
