1. (a) The department will provide application forms for persons to use in applying for a permit, certificate of approval, or other service under this chapter. An application form submitted under this chapter, and any document accompanying an application form, must be completed in ink.

2. (b) Along with an application form, the department will provide to an applicant for a permit

   1. (1) a copy of the definitions of the following terms, as set out in 13 AAC 30.900:

      1. (A) alcohol treatment program;
      2. (B) convicted;
      3. (C) felony;
      4. (D) resident; and
      5. (E) substance abuse treatment program;
   2. (2) a concise summary of where, when, and by whom a handgun can be carried under state and federal law; and
   3. (3) written notice that, for purposes of AS 18.65.705(4) and (5), the date of conviction is the date sentence was imposed or imposition of sentence was suspended under AS 12.55.085 or a similar procedure in another jurisdiction.
