1. (a) An application for initial licensure by examination as a psychologist must be submitted on a form provided by the department with

   1. (1) the applicable fees specified in 12 AAC 02.330;
   2. (2) official transcripts from all undergraduate and graduate schools attended by the applicant;
   3. (3) repealed 5/18/85;
   4. (4) a complete vita from the date of high school graduation to the time of application, including dates and places of residency; and
   5. (5) repealed 5/18/85;
   6. (6) five letters of recommendation; three from licensed psychologists, members of the American Psychological Association, or diplomates of the American Board of Professional Psychology; and two from other persons not related to the applicant;
   7. (7) repealed 12/8/2005.

2. (b) An application for initial licensure by examination as a psychological associate must be submitted on a form provided by the department with

   1. (1) the applicable fees specified in 12 AAC 02.330;
   2. (2) official transcripts from all undergraduate and graduate schools attended by the applicant;
   3. (3) a complete vita from the date of high school graduation to the time of application, including dates and places of residency;
   4. (4) proof of a practicum from an accredited school with an approved program in the area in which licensure is requested; and
   5. (5) repealed 3/27/98;
   6. (6) the recommendations required by AS 08.86.162(3).

3. (c) Before the board will issue a psychological associate license by examination, the applicant shall submit proof of

   1. (1) completion of the supervised experience required under AS 08.86.160(a)(3), as defined in 12 AAC 60.070(c); and
   2. (2) passing the required examination in 12 AAC 60.110(g).

4. (d) Before the board will issue a psychology license by examination, the applicant shall submit proof of

   1. (1) completion of the supervised experience required in AS 08.86.130(a)(3), as described in 12 AAC 60.070(b); and
   2. (2) passing the required examination in 12 AAC 60.110(g).
