1. (a) An applicant for renewal of a license to practice optometry shall submit, on a form provided by the department, a statement of the continuing education instruction required by 12 AAC 48.200 - 12 AAC 45.220 that the applicant has completed during the concluding license period.
2. (b) Falsification of any written evidence submitted under this section or 12 AAC 02.960 is unprofessional conduct and constitutes grounds for license reprimand, revocation, or suspension.
