Alaska Admin. Code tit. 12, § 44.290 – Application for examination

(a) An applicant for license by examination in this state shall
1. (1) submit a completed application, on a form provided by the department, for approval to take the licensing examination; the completed application must include
  1. (A) for identification purposes, the personal identification information requested on the application form;
  2. (B) an attestation that the applicant graduated from or is eligible to graduate from a nursing education program approved by the board;
  3. (C) the applicant's licensing and examination history;
  4. (D) information on the applicant's physical and mental health related to the grounds for license denial, suspension, or revocation in AS 08.68.270;
  5. (E) information on any criminal convictions related to the grounds for license denial, suspension, or revocation in AS 08.68.270;
  6. (F) the applicant's notarized signature certifying that the information in the application is correct to the best of the applicant's knowledge;
  7. (G) the applicant's fingerprint information described in 12 AAC 44.319(a);
2. (2) pay the application fee, the fingerprint processing fee, and the license fee specified in 12 AAC 02.280;
3. (3) provide to the board, if the applicant is
  1. (A) a graduate of a foreign pre-licensure education program that is not taught in English
    1. (i) verification that the applicant has passed an English proficiency examination that includes components on reading, speaking, writing, and listening;
    2. (ii) an evaluation of the applicant's nursing education by the Commission on Graduates of Foreign Nursing Schools Credentials Evaluation Service, including a full education, course-by-course report that indicates the applicant's nursing education was taught in English; or
    3. (iii) an official International Commission on Healthcare Professions (ICHP) certificate that verifies that the applicant has successfully completed the VisaScreen: Visa Credential Assessment Service;
  2. (B) a practical nurse applicant who is a United States military servicemember on active duty or reserve status, or discharged by means other than a dishonorable discharge not more than five years from the date of application, who has completed a nursing education program provided by the United States military that is equivalent to an accredited practical nurse education program under AS 08.68.170
    1. (i) a complete certified transcript of the applicant's military education, submitted directly to the board from the appropriate military service branch;
    2. (ii) verification of employment on a form provided by the board that the applicant has, within the last five years, worked in the United States military in a nursing role for which the applicant received training equivalent to an accredited practical nurse education program; and
    3. (iii) an attestation that the applicant is familiar with the licensed practical nurse scope of practice set out under 12 AAC 44.136;
4. (4) repealed 4/27/83;
5. (5) provide a nursing program verification form; the nursing program verification form must be completed by an official of the school of nursing attended and submitted directly to the department from the school of nursing; and
6. (6) provide, at the written request of the board, additional information that is necessary to demonstrate that the applicant has met the licensing requirements of AS 08.68 and this chapter.
(b) The application, fees, and all supporting documents required under (a) of this section must be on file with the department before the applicant will be approved to take the licensing examination.
(c) An applicant who is approved to take the licensing examination is eligible for examination for one year after the date the department sends a notice of approval to take the NCLEX to the testing company. An applicant who does not take the examination within that year shall reapply for approval under (a) of this section and submit the application fee and fingerprint processing fee.
