An applicant for a license by credentials shall submit
- (1) a completed application on a form provided by the department;
- (2) the applicable application and license fees established in 12 AAC 02.200;
- (3) verification of a current license to practice as a dispensing optician issued by another licensing jurisdiction that meets the requirements of AS 08.71.145; acceptable verification may include documentation that demonstrates the applicant passed the applicable examination under AS 08.71.090;
- (4) repealed 6/22/2003;
- (5) verification of work experience or training equivalent to that required by AS 08.71.110(a)(2); and
- (6) a complete authorization for release of records form.
