The board may, upon its own motion or upon the request of any interested person, approve a course of instruction upon receipt of

1. (1) the name of the college or university sponsoring the course;
2. (2) the name of the accredited program and faculty member presenting the course;

3. (3) a course outline that verifies inclusion of the subjects and procedures required under

   1. (A) 12 AAC 28.340 if certification to administer local anesthesia is sought; or
   2. (B) 12 AAC 28.345, if certification to administer nitrous oxide sedation is sought; and
4. (4) an explanation of the evaluation procedures used to determine successful completion of the course.
