Alaska Admin. Code tit. 12, § 12.035 – Application, renewal, and termination of a telemedicine business registration

(a) To be registered on the telemedicine business registry, a business that performs telemedicine services shall pay the applicable fee as established in AS 44.33.381 and
1. (1) apply for registration as part of the initial application for a new business license under 12 AAC 12.030 or the application for renewal of a business license under 12 AAC 12.040, and complete the telemedicine business registration section of the application; or
2. (2) submit a telemedicine business registration form to the department, if the applicant is the holder of an existing business license not previously registered to provide telemedicine services.
(b) A telemedicine business registration is subject to renewal every two years, together with the corresponding business license, and expires on the same expiration date. To renew a telemedicine business registration, the business shall
1. (1) submit a business license renewal application in accordance with 12 AAC 12.040;
2. (2) elect to maintain the telemedicine business registration; and
3. (3) pay the applicable fee as established in AS 44.33.381.
(c) If a business terminates the performance of telemedicine services, the business shall notify the department and request that the department remove the business from the telemedicine business registry.
