Verification of licensed work experience or training of an applicant required under this chapter must be certified or notarized documentation supplied to the board by

1. (1) another jurisdiction's licensing board;
2. (2) a licensed school or shop;
3. (3) a licensed instructor;
4. (4) a licensed hairdresser, esthetician, barber, non-chemical barber, body piercing practitioner, tattooing practitioner, or permanent cosmetic coloring practitioner who sponsored an apprentice and maintained the required records under 12 AAC 09.190; or
5. (5) an individual who has direct personal knowledge of the applicant's work experience hours.
