An applicant for a license by waiver of examination to practice tattooing or permanent cosmetic coloring shall apply on a form provided by the department. The application must include

1. (1) payment of the applicable fees established in 12 AAC 02.140;

2. (2) verification that the applicant

   1. (A) holds a current license to practice tattooing or permanent cosmetic coloring in another state; and
   2. (B) is qualified for licensure to practice tattooing or permanent cosmetic coloring in that state by passing a written examination;

3. (3) verification that the applicant has successfully completed training equivalent to that required for completion of courses certified by the American Red Cross, the American Heart Association, or a similar organization approved by the board, and has been issued a current card, in

   1. (A) cardiopulmonary resuscitation (CPR); and
   2. (B) blood borne pathogens; and
4. (4) verification that the applicant has completed training equivalent to that described in AS 08.13.082(d) and 12 AAC 09.169(a) - (c).
