1. (a) A well is considered to be completed 30 days after

   1. (1) drilling and testing operations have ceased and the well is capable of producing a geothermal resource;
   2. (2) the well has begun to produce geothermal resources, unless drilling operations are resumed before the end of the 30 day period; or
   3. (3) the well is plugged and abandoned.
2. (b) All required logs, records and test results must be submitted to the commissioner within 60 days after completion of drilling operations.

3. (c) A completion report must be submitted to the commissioner within 60 days after completion of drilling operations. The completion report must provide the following information:

   1. (1) the actual depth of the well;
   2. (2) the actual depth to zone of completion;
   3. (3) the actual bottom-hole location for a directionally drilled well;
   4. (4) method of completion or abandonment; and
   5. (5) any information on drilling difficulties or unusual circumstances encountered which would be helpful in assuring future safety of operations or protection of the environment.
